Summary:
The Population Health and VBP Program Manager role supports the organization's commitment to value-based and patient-centered health care.
They are responsible for overseeing patient navigators and coordinating the workflow in the department working with payors and vendors to meet established quality measures for all value-based payment programs.
Duties and Responsibilities include the following.
Other duties may be assigned.
Responsible for all Value Based Payment opportunities and the potential financial benefits of meeting all requirements for each plan, on an annual basis.
Provides a summary of all VBP plans from a clinical perspective and requirements.
Reviews workload versus payment opportunity and analyzes with CEO and practice.
Responsible for the build in Medent (Practice Electronic Medical Record System), if required, and operationalizing registries in Medent to meet the needs of the practice.
Responsible for development of implementation program to include training of nursing staff and operationalizing quality metrics.
Responsible for identifying resources needed to implement selected VBP programs.
Provides monthly reporting to CEO regarding the current status of VBP programs being pursued in the practice.
Ensures AWV is completed for all eligible patients.
Develops practice workflows to insure success in the selected programs.
Participates in maintaining PCMH Level 3 certification for the practice, annually.
Works to achieve maximum success in Excellus ACQA quality metrics.
Captures 90% of all available reimbursement for VBP programs selected by the partners.

Desired
Experience:
1-4 years' experience of managed care, quality improvement and/ or provider contracting.
Extensive knowledge of HEDIS quality metrics, Star Ratings, Value Based Contracting.
Knowledge of value-based program strategies, cost of care initiatives, and performance analytics Knowledge of practice workflows, industry best practices, advanced value-based contract terms and settlement calculations Collaboration with internal and external stakeholders.

Education Requirements:
Bachelor's degree in a health-related field and work history in a health care setting.
Experience Requirements:
Relevant experience in health care, population health, care management, or related support role.
Excellent verbal/written communication, interpersonal, problem-solving, and critical thinking skills.
Proficiency in Microsoft Office (Excel, Access, Word).
Ability to work autonomously and adjust to changing needs as the role develops.
Computer Skills:
To perform this job successfully, an individual should have knowledge of Electronic Health Records programs, word processing software, spreadsheet software, and internet software.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
