The navigator facilitates patient care through disease management and health promotion.
The work is data-driven and utilizes a population health approach to identify and assist patients on all parts of the health care continuum.
The navigator is responsible for overseeing and coordinating compliance of quality measures for entire value-based payor population and providing reporting on data relevant to the assigned patient population.
Interacts with patients, staff, and providers to improve communication about upcoming care opportunities needed at the patients next encounter/visit.
Utilizes EMR effectively to communicate with the health care team via triages.
Maintains current knowledge of NCQA HEDIS measures and gap closure measures by attending training and working with the concurrent insurance company/appointed point person to ensure overall office success The candidate must be able to function in a fast-paced environment, successfully prioritize responsibilities and multitask.

POSITION REQUIREMENTS:
Education Requirements:
High school diploma or general education degree (GED) required; Bachelor's degree in a health-related field and work history in a health care setting preferred.
Certified Professional Coder (CPC) is preferred.
Minimum of two years relevant experience in primary care, population health, care management or related support role or equivalent combination of education and experience.
Experience Requirements:
Proficiency working with EMR.
Prior experience with MEDENT Electronic record is preferable.
Must have strong knowledge of Medical Terminology and Medical Records/Reports.
Working knowledge of CPT and ICD9/10 codes, HIPAA, billing and insurance regulations, medical terminology, insurance benefits and appeal processes.
Proficient computer operation experience as well as experience in Microsoft Word, Excel, and Outlook.
Strong customer service skills including phone contact.
Excellent verbal/written communication, interpersonal, problem-solving, and critical thinking skills.
Ability to work autonomously and adjust to changing needs as the role develops.

The check will bounce and the victim is left responsible.
